Adrenocortical suppression and recovery after continuous hypnotic infusion: etomidate versus its soft analogue cyclopropyl-methoxycarbonyl metomidate

Introduction
Etomidato não é administrado em infusão contínua para manutenção anestésica ou sedação porque resulta na supressão profunda e persistente da síntese de esteróide adrenocortical com consequências potencialmente letais em pacientes criticamente enfermos. Os autores mostram uma hipótese que o uso de análogos do etomidade que são rapidamente metabolizados poderiam ser desenvolvidos e sem produzir disfunção adrenocortical persistente, mesmo após a infusão contínua prolongada. Esperam que esses agentes também possam fornecer mais rápida e previsível recuperação anestésica. Foi então desenvolvido o  ciclopropil metoxicarbonil metomidato (CPMM). Após encerrar infusões contínuas de 120 minutos, as recuperações hipnóticas e encefalográficas ocorrem em quatro minutos. O objetivo deste estudo foi avaliar a função adrenocortical durante e após 120 minutos de infusão contínua de CPMM e comparar os resultados com aqueles obtidos com etomidato.
Methods
Ratos dexamethasone-suppressed foram distribuídos aleatoriamente em um grupo Etomidato, grupo CPMM ou grupo de controle. Ratos no do grupo etomidato e grupo CPMM receberam 120 minutos de infusão contínua de etomidato ou CPMM, respectivamente. Os ratos do grupo de controle não receberam nenhum hipnótico. No primeiro estudo, a função adrenocortical durante a infusão do hipnótico foi avaliada por administração de hormônio adrenocorticotrópico (ACTH), 90 minutos após o início da perfusão do hipnótico e medindo-se as concentrações de corticosterona no plasma no final da infusão 30 minutos mais tarde. No segundo estudo, recuperação adrencorticóide após a infusão do hipnótico foi avaliada pela administração de ACTH a cada 30 minutos após o término da infusão e pela medição das concentrações plasmáticas de corticosterona 30 minutos após cada dose de ACTH.
 Results
Durante a infusão do hipnótico, concentrações séricas de ACTH estimulado foi significativamente menor nos grupos CPMM and etomidato em relação ao grupo controle (100 +/- 64 ng/ml and 33 +/- 32 ng/ml versus 615 +/- 265 ng/ml, respectivamente). Após a infusão do hipnótico, as concentrações séricas de ACTH estimulado recuperou-se aos valores controle em 30 minutes no grupo CPMM mas manteve-se suprimidopor mais de 3 horas no grupo etomidato em relação ao controle.

Conclusions

Ambos CPMM e etomidato suprimem a função adrenocortical durante a infusão contínua. Entretanto, a recuperação ocorre significativamente mais rápido com a infusão de CPMM.

Remifentanil e propofol vs Midazolan e fentanil: menor tempo de VM e mesmos custos

Sedation in the intensive care unit with remifentanil/propofol versus midazolam/fentanyl: a randomised, open-label, pharmacoeconomic trial


Introduction
Remifentanil is an opioid with a unique pharmacokinetic profile. Its organ-independent elimination and short context-sensitive half time of 3 to 4 minutes lead to a highly predictable offset of action. We tested the hypothesis that with an analgesia-based sedation regimen with remifentanil and propofol, patients after cardiac surgery reach predefined criteria for discharge from the intensive care unit (ICU) sooner, resulting in shorter duration of time spent in the ICU, compared to a conventional regimen consisting of midazolam and fentanyl. In addition, the two regimens were compared regarding their costs.

Methods
In this prospective, open-label, randomised, single centre study, a total of 80 patients (18 to 75 years old), who had undergone cardiac surgery, were postoperatively assigned to one of two treatment regimens for sedation in the ICU for 12 to 72 hours. Patients in the remifentanil/propofol group received remifentanil (6- max. 60 μg kg-1 h-1; dose exceeds recommended labelling). Propofol (0.5 to 4.0 mg kg-1 h-1) was supplemented only in the case of insufficient sedation at maximal remifentanil dose. Patients in the midazolam/fentanyl group received midazolam (0.02 to 0.2 mg kg-1 h-1) and fentanyl (1.0 to 7.0 μg kg-1 h-1). For treatment of pain after extubation, both groups received morphine and/or non-opioid analgesics.

Results
The time intervals (mean values ± standard deviation) from arrival at the ICU until extubation (20.7 ± 5.2 hours versus 24.2 h ± 7.0 hours) and from arrival until eligible discharge from the ICU (46.1 ± 22.0 hours versus 62.4 ± 27.2 hours) were significantly (p < 0.05) shorter in the remifentanil/propofol group. Overall costs of the ICU stay per patient were equal (approximately €1,700 on average).

Conclusion
Compared with midazolam/fentanyl, a remifentanil based regimen for analgesia and sedation supplemented with propofol significantly reduced the time on mechanical ventilation and allowed earlier discharge from the ICU, at equal overall costs.

Sugammadex

Sugammadex: Another Milestone in Clinical Neuromuscular Pharmacology


Sugammadex is a revolutionary investigational reversal drug currently undergoing Phase III testing whose introduction into clinical practice may change the face of clinical neuromuscular pharmacology. A modified -cyclodextrin, sugammadex exerts its effect by forming very tight water-soluble complexes at a 1:1 ratio with steroidal neuromuscular blocking drugs (rocuronium vecuronium pancuronium). During rocuronium-induced neuromuscular blockade, the IV administration of sugammadex creates a concentration gradient favoring the movement of rocuronium molecules from the neuromuscular junction back into the plasma, which results in
a fast recovery of neuromuscular function. Sugammadex is biologically inactive, does not bind to plasma proteins, and appears to be safe and well tolerated.
Additionally, it has no effect on acetylcholinesterase or any receptor system in the body. The compound’s efficacy as an antagonist does not appear to rely on renal excretion of the cyclodextrin-relaxant complex. Human and animal studies have demonstrated that sugammadex can reverse very deep neuromuscular blockade induced by rocuronium without muscle weakness. Its future clinical use should decrease the incidence of postoperative muscle weakness, and thus contribute to increased patient safety.
Sugammadex will also facilitate the use of rocuronium for rapid sequence induction of anesthesia by providing a faster onset-offset profile than that seen with 1.0 mg/kg succinylcholine. Furthermore, no additional anticholinesterase or anticholinergic drugs would be needed for antagonism of residual neuromuscular blockade, which would mean the end of the cardiovascular and other side effects of these compounds. The clinical use of sugammadex promises to eliminate many of the shortcomings in our current practice
with regard to the antagonism of rocuronium and possibly other steroidal neuromuscular blockers.

Goodbye Suxamethonium!

No drugs in anaesthesia are more problematic than suxamethonium. Yet, no drugs have survived as
suxamethonium does in spite of crisis after crisis, and attempt after attempt at its replacement. For
decades, suxamethonium has taught us neuromuscular pharmacology and provided us with an
encyclopaedia of side effects, while benefiting millions and millions of our anaesthetised patients.
With the arrival of sugammadex, it finally appears that suxamethonium can be retired. Suxamethonium
has done its job and seen its days! The present review is intended to offer a eulogy for
suxamethonium.

Morfina vs fentanil em cirurgia cardíaca

Interessante a análise do uso da morfina, comparando-a ao fentanil em cirurgia cardíaca!

Conclusão do artigo: Em pacientes submetidos a cc eletiva, com cec, as medidas de qualidade de vida pós-op e controle da dor na recuperação foram melhores com morfina 40 mg quando administradas intra-op como parte de anestesia balanceada com isoflurano do que comparado com fentanil.

Abstract

BACKGROUND: Experimental and clinical data suggest that morphine possesses
unique cardioprotective and antiinflammatory properties. In this clinical
investigation, we sought to determine whether the choice of intraoperative
opioid (morphine or fentanyl) influences early recovery after cardiac
surgery.

METHODS: Ninety patients undergoing cardiac surgery with cardiopulmonary
bypass were randomized to receive either morphine (40 mg) or fentanyl (600
µg) as part of a standardized opio id-isoflurane anesthetic. Quality of
recovery was assessed using the QoR-40 questionnaire administered
preoperatively and daily on postoperative days 1-3. During the first three
postoperative days, pain was measured using a 100-mm visual analog scale,
and the use of IV and oral pain medications (morphine or
acetaminophen/hydrocodone) was quantified. Hemodynamic variables, duration
of tracheal intubation, postoperative febrile reactions, organ morbidities,
and intensive care unit (ICU) and hospital length of stay were evaluated.

RESULTS: Compared with patients given fentanyl, those receiving morphine had
higher global QoR-40 scores on postoperative days 1 (173 vs 160, P <
0.0001), 2 (174 vs 164, P < 0.0001), and 3 (177 vs 167, P < 0.001).
Differences between the groups were observed in the QoR-40 dimensions of
emotional state, physical comfort, and pain (all P < 0.01-0.0001).
Postoperative visu al analog scale pain scores, use of pain medication in the
ICU and surgical ward, and postoperative febrile reactions were reduced
significantly in the morphine group (all P < 0.01). No differences between
the groups were noted in duration of tracheal intubation, ICU and hospital
length of stay, or postoperative complications.

CONCLUSIONS: In patients undergoing elective cardiac surgery with
cardiopulmonary bypass, postoperative quality-of-life measures and pain
control during recovery were enhanced when morphine (40 mg) was administered
intraoperatively as part of a balanced anesthetic technique compared with
fentanyl.
